They are no longer accepting the affidavit and you are required to get your fingerprints.

The ones that are starred are forms that have been on the district website, but something new we have to turn in along with our other clearances. Please read Act 24 and PT Policy 824. PT Policy 824 explains adult and student boundaries that we all must follow when interacting with students in any setting. The form does talk about a training video. We have asked about this, but are still seeking information about this training. For now, please review the forms and sign.
Many of you have already turned in your clearances and may only need to turn in your fingerprints and forms Act 24 and Policy 824.
Mrs. Gutwald is going through everyone's files to confirm who needs forms turned in.
If you have not already gotten your clearances, now is the time!
The approval date must be within a year of submitting to the PT Drama Guild.
We are always in need of volunteers and we are required to follow school policy with turning in clearances
before being able to work with the children.

Once you obtain your clearances, please present your clearances to Mrs. Karrie Gutwald,
