Pennsylvania State Police Request for Criminal Records Check - Act 34
Department of Public Welfare Child Abuse History Clearance - Act 151
Federal Criminal History Record / Fingerprinting or Affidavit - Act 114
If you have lived in Pennsylvania for >10 years and can attest to the Affidavit below, you can sign the
PTSD Volunteer Clearance Affidavit form instead of the FBI Fingerprinting (please see details below)
Please see the detailed information below as the clearances are now FREE at this time for volunteers.
Once you obtain your clearances, please present your clearances to Mrs. Karrie Gutwald,
our PTDG Business Manager who will make a copy of the clearances and keep on file.
